ABSTRACT

Resumo Diversas manobras já foram descritas para o acesso ao segmento distal cervical da artéria carótida interna ou à bifurcação carotídea alta; entretanto, há divergências na sistematização dessas técnicas. O objetivo deste estudo é revisar as técnicas descritas e propor um protocolo prático que auxilie na seleção da técnica mais adequada para cada caso. Para isso, foi realizada uma busca nas bases de dados PubMed Central, Biblioteca Virtual em Saúde e SciELO por artigos sobre o tema, em língua inglesa ou portuguesa, publicados entre os anos de 1980 e 2021. Entre as manobras descritas, parece razoável que as duas etapas iniciais sejam a abordagem ao músculo esternocleidomastóideo, seguida pela secção/retração do ventre posterior do músculo digástrico. Caso necessário, a subluxação mandibular temporária unilateral é um recurso adicional e preferível à divisão do aparato estiloide, devido ao menor potencial de morbidade. Exposições ainda mais amplas podem ser obtidas com as osteotomias mandibulares.


Abstract Several different maneuvers have been described for obtaining access to the distal cervical segment of the internal carotid artery or to a high carotid bifurcation. However there are different approaches to systematization of these techniques. The objective of this study is to review the techniques described and propose a practical protocol to support selection of the most appropriate technique for each case. The review is based on the results of database searches on PubMed Central, the Virtual Health Library (BVSalud), and SciELO for articles on the subject published in English or Portuguese from 1980 to 2021. Among the different maneuvers described, it appears reasonable that the first two steps should be to obtain access at the sternocleidomastoid muscle, followed by section or retraction of the digastric muscle posterior belly. If needed, temporary unilateral mandibular subluxation is an additional resource that is preferable to division of the styloid apparatus process, because of its lesser potential for morbidity. Even wider exposure can be obtained using mandibular osteotomies.

ABSTRACT

RESUMEN El ameloblastoma es un tumor odontogénico benigno, localmente agresivo y recidivante, con predilección por la región posterior de la mandíbula. Se caracteriza por su agresividad local con muy baja tendencia a metastizarse. El objetivo fue reportar el caso clínico de un paciente con ameloblastoma multiquístico derecho, tratado a través de hemimandibulectomía. Se presentó un paciente masculino, de 44 años de edad, que refirió aumento de volumen del lado derecho de la mandíbula desde hacía aproximadamente un año, acompañado también de otros síntomas, atendido en el Servicio de Cirugía Maxilofacial del Hospital Militar Principal/Instituto Superior, en Luanda, Angola. Los estudios imagenológicos incluyeron radiografía panorámica y tomografía axial computarizada. El diagnóstico clínico patológico fue de ameloblastoma multiquístico. Este tipo de tumor requiere de un adecuado diagnóstico sobre la base de la presentación clínica, localización, tamaño, edad y tipo histológico; de ahí la importancia de conocer las características clínicas e imagenológicas, pues el tratamiento conlleva gran dificultad (AU).


ABSTRACT Ameloblastoma is a benign odontogenic tumor, locally aggressive and recidivist with predilection for back of the jaw, characterized by local aggressiveness and low tendency to metastasize. The aim was reporting the clinical case of a patient with right multicystic ameloblastoma treated through hemimandibulectomy. We presented a male patient aged 44 years, who referred a volume increase of the jaw right side for around a year, accompanied also by other symptoms; he attended the Maxillofacial Surgery Service of the Main Military Hospital/High Institute of Luanda, in Angola. The image studies included panoramic radiography and computerized axial tomography the clinical pathological diagnosis was multicystic ameloblastoma. This kind of tumor requires an adequate diagnosis based on the clinical presentation, location, size, age and histological kind, therefore the importance of knowing the clinical and image characteristics, because the treatment is very difficult (AU).

RESUMEN Introducción: Las anomalías dentofaciales son trastornos del crecimiento dental y facial que afectan tanto a niños como a adultos. Objetivo: Presentar un caso clínico, en que en la búsqueda de armonía, por una anomalía dentofacial, se aplicaron principios del manejo multidisciplinario de cirugía ortognática y de otoplastia. Caso clínico: Paciente masculino de 18 años de edad que refiere inconformidad estética y en el que, mediante el análisis de modelos de estudio, mediciones de las telerradiografías indicadas y los hallazgos del examen físico, se llegó al diagnóstico de prognatismo mandibular con exceso vertical del mentón. Se ejecuta tratamiento ortodóncico-quirúrgico, realizándose osteotomía sagital mandibular y mentonoplastia de reducción de altura. Se emplearon como medios de fijación interna los tornillos bicorticales en la zona de ángulo mandibular y miniplacas en la región del mentón. Tres meses después se realizó otoplastia, para corregir la presencia de orejas prominentes, con el logro de un resultado funcional y estético satisfactorio. Conclusiones: Con la aplicación de los principios del manejo multidisciplinario de cirugía ortognática, combinada con la realización de una otoplastia, los resultados fueron positivos. Se alcanzó el objetivo de brindar armonía facial, con mejoría funcional y estética, y una alta satisfacción del paciente(AU)


ABSTRACT Introduction: Dentofacial anomalies are dental and facial growth disorders affecting children and adults alike. Objective: Present a clinical case of dentofacial anomaly in which principles of the multidisciplinary management of orthognathic surgery and otoplasty were applied to achieve harmony. Case report: A male 18-year-old patient reports esthetic dissatisfaction. Analysis of study models, measurements taken by teleradiography and findings of the physical examination led to the diagnosis of mandibular prognathism with a vertically extended chin. Orthodontic-surgical treatment was performed, consisting in sagittal mandibular osteotomy and height reduction mentoplasty. Bicortical screws were used for internal fixation in the mandibular angle area and miniplates in the chin region. Otoplasty was performed three months later to correct the presence of protruding ears, achieving satisfactory functional and esthetic results. Conclusions: Application of the principles of the multidisciplinary management of orthognathic surgery, combined with the conduct of otoplasty, led to positive results. The objective of achieving facial harmony was fulfilled, alongside functional and esthetic improvement and high patient satisfaction(AU)

ABSTRACT

ABSTRACT: Radiotherapy applies ionizing radiation at predetermined doses for a limited period of time in order to destroy tumors. The oral cavity, which has a high rate of cell renewal, is affected by the side effects of radiotherapy including osteoradionecrosis (ORN). This condition occurs due to irradiated bone tissue that becomes devitalized and exposed in the oral cavity. Conservative therapies are recommended for ORN lesions that are not extensive or in an early stage. Surgical intervention is necessary for extensive areas affected by necrosis. This study reports a case of ORN in the left mandibular body which resulted in a pathological fracture. The treatment consisted of segmental mandibulectomy and the use of a reconstruction plate. Also, low-level laser therapy around bone exposure was performed. After surgery, the patient underwent 6 months of follow-up and was satisfied with the outcome. However, the patient died before control of ORN was achieved due to a heart attack.


RESUMEN: La radioterapia aplica radiación ionizante a dosis predeterminadas durante un período de tiempo limitado para destruir tumores. La cavidad oral, que tiene una alta tasa de renovación celular, se ve afectada por los efectos secundarios de la radioterapia, incluida la osteorradionecrosis (ORN). Esta condición se produce debido al tejido óseo irradiado que se desvitaliza y expone en la cavidad oral. Se recomiendan terapias conservadoras para las lesiones de ORN que no son extensas o en una etapa temprana. La intervención quirúrgica es necesaria para áreas extensas afectadas por necrosis. Este estudio reporta un caso de ORN en el cuerpo mandibular izquierdo que resultó en una fractura patológica. El tratamiento consistió en mandibulectomía segmentaria y el uso de una placa de reconstrucción. Además, se realizó una terapia con láser de bajo nivel alrededor de la exposición ósea. Después de la cirugía, el paciente se sometió a 6 meses de seguimiento y quedó satisfecho con el resultado. Sin embargo, el paciente falleció antes de que se lograra el control de ORN debido a un ataque cardíaco.

ABSTRACT

Pierre Robin sequence is a set of congenital conditions characterized by the presence of micrognathia, glossoptosis, cleft palate and obstruction of the airways. The latter can lead to many other complications such as respiratory difficulties, apnea, weight loss and even death. Currently, mandibular distraction, or the lengthening of the mandibular bone, is the most common surgical procedure used to correct a retracted tongue and the airway obstruction it produces in patients with mandibular hypoplasia. The present paper reports the case of a 26-day-old male patient, who presented obstruction on the upper respiratory tract, mandibular retromicrognathia, and retracted tongue and cleft palate, all conditions characteristic to Pierre Robin sequence. The patient also had a medical record of orotracheal intubation due to respiratory difficulties. The subject underwent mandibular distraction surgery with a horizontal vector, which resulted in a satisfactorily cleared airway.


La secuencia de Pierre Robin es una afección congénita caracterizada por la presencia de micrognatia, glosoptosis, paladar hendido y obstrucción de la vía aérea, siendo ésta una de las principales características de la cual derivan varias complicaciones, entre ellas, dificultad respiratoria, apnea, pérdida de peso y hasta la muerte. En la actualidad la distracción ósea mandibular es la técnica quirúrgica de elección, que tiene como finalidad el alargamiento mandibular corrigiendo la posición posterior de la lengua, con la consecuente desobstrucción de la vía aérea en pacientes con hipoplasia mandibular. Se reporta caso clínico de paciente masculino con 26 días de nacido, que presentó obstrucción de la vía aérea superior, retromicrognatismo mandibular, retracción de la lengua y hendidura palatina, relacionado con la secuencia de Pierre Robin, con antecedentes de intubación orotraqueal por dificultad respiratoria, al cual se le realizó distracción ósea mandibular con vector horizontal, destacando resultados satisfactorios en la resolución de la obstrucción de la vía aérea.

INTRODUCTION Malignant neoplasms of the head and neck, due to its anatomical location, can cause significant alterations in vital functions related to feeding, communication and social interaction of the affected patients. Objective To analyze the quality of life of patients with advanced malignant neoplasms of the oral cavity and submitted to radical operations with curative intent. Material and methods 47 patients with oral cavity squamous cell carcinoma (SCC), in stages III and IV, underwent surgical treatment with segmental mandibulectomy and complementary radiotherapy. The patients were submitted to the quality of life questionnaires after a minimum time of six months after the surgical treatment. Results Of the 183 patients, only 47 (25.7%) were able to answer the questionnaire and were included as the sample of the study. The majority of patients selected were male (39; 82.9%). The mean age was 64.4 years. The majority of the patients presented clinical stage IV (83%) and were submitted to adjuvant radiotherapy (95.4%). The mean score obtained after the questionnaires were applied was 64.6. The worst scores were found in swallowing and chewing. Conclusion There were no statistically significant differences in the domains of quality of life between the two groups studied (with bone reconstruction versus no bone reconstruction). Patients interviewed 2 years or more after treatment presented higher scores (p = 0.02).


RESUMO INTRODUÇÃO As neoplasias malignas de cabeça e pescoço, pela própria localização anatômica, podem acarretar alterações significativas em funções vitais relacionadas à alimentação, comunicação e interação social dos indivíduos afetados. OBJETIVO Analisar a qualidade de vida dos pacientes com neoplasias malignas avançadas de cavidade oral, submetidos a operações radicais com intenção curativa. MATERIAL E MÉTODOS 47 pacientes portadores de carcinoma espinocelular de cavidade oral, em estádios III e IV, foram submetidos ao tratamento cirúrgico com mandibulectomia segmentar e radioterapia complementar. Os pacientes foram submetidos ao teste de qualidade de vida após o tempo mínimo de seis meses do tratamento cirúrgico. RESULTADOS Dos 183 pacientes, com apenas 47 (25,7%) foi possível a realização da entrevista, compondo estes a amostra para o estudo. A maioria dos pacientes do grupo selecionado era do sexo masculino, total de 39 homens (82,9%). A idade média foi de 64,4 anos. A maioria dos pacientes apresentava estadiamento clínico IV (83%), sendo submetidos à radioterapia adjuvante (95,4%). A média do escore obtido após a avaliação dos questionários foi de 64,6. Os piores escores foram encontrados nos quesitos deglutição e mastigação. CONCLUSÃO Não houve diferenças estatisticamente significativas nos domínios de qualidade de vida entre os dois grupos estudados (com reconstrução óssea versus sem reconstrução óssea). Pacientes entrevistados dois anos ou mais após o tratamento apresentaram escores superiores (p=0,02).

ABSTRACT

The formation of a new joint between a pathologically enlarged coronoid process and the body of the malar bone is known as Jacob's disease. hyperplasia of the coronoid process was first described in 1853 by von Langenbeck, and it was not until 1899 when Oscar Jacob described the disease that was named after him. Jacob's disease is an uncommon entity with only a few cases documented in the literature. the condition manifests at first with progressive limitation of the oral opening and facial asymmetry. the pain is infrequent and mainly affects young patients. temporal muscle hyperactivity, cranial trauma, chronic displacement of the ipsilateral temporomandibular joint, endocrine stimuli and genetic alterations have been postulated as possible factors. the definitive diagnosis is by histopathology and it is necessary that bone hyperplasia is confirmed, as well as the presence of cartilage and synovial capsule forming the new joint between the malar bone and the coronoid process. we present a 10-year-old patient with a history of childhood trauma in the left preauricular region. it presented to our service with a history of progressive limitation of the oral opening. computed tomography (CT) revealed an elongation of the bilateral coronoid process, in contact with homolateral zygomatic bone, causing its deformation. surgery under general anesthesia was performed through the intraoral vestibular route. histopathology confirmed the diagnosis of Jacob's disease. we review the literature regarding the etiology, pathogenesis, clinical characteristics, diagnosis and treatment of this condition.

ABSTRACT

Abstract Clear cell odontogenic carcinoma (CCOC) is a rare odontogenic tumor of the jaws, histologically characterized by the presence of agglomerates of cells with eosinophilic cytoplasm. The patient, a 62-year-old Caucasian woman, presented an intraosseous lesion in the mandibular symphysis. A clinical examination revealed a discrete volumetric increase with a hard consistency, palpable to extraoral and intraoral examinations. Imaging studies revealed an extensive radiolucent area, without defined limits, extending from the region of the right second premolar to the left canine. Incisional biopsy analysis indicated a diagnosis of CCOC. The treatment proposed was segmental resection of the mandible with a safety margin. After six months without recurrence, definitive mandibular reconstruction was performed using an iliac crest graft, followed by rehabilitation with implant-supported denture after five months. After three years of post-resection follow-up, the patient has shown no evidence of recurrence or metastasis. She continues to be under follow-up. To conclude, CCOC must be considered a malignant tumor with aggressive behavior. Previous studies have shown that resection with free margins is a treatment with a lower rate of recurrence. Nevertheless, long-term follow-up is necessary for such patients.

Introdução: O descontentamento com a estética facial é considerado o fator motivador mais frequente na procura pela cirurgia ortognática, visto que este é o procedimento indicado nos casos de severas discrepâncias dentoesqueléticas em pacientes adultos. A anatomia das vias aéreas superiores (VAS) permite que fatores como obesidade, hipotonia muscular e deficiência mandibular favoreçam sua obstrução, podendo gerar a Apneia Obstrutiva do Sono (AOS), caracterizada por episódios recorrentes de obstrução parcial ou completa das VAS durante o sono. As cirurgias de avanço bimaxilar estão associadas ao aumento do espaço aéreo, no entanto, as alterações morfológicas e volumétricas ainda não são bem conhecidas. Objetivos: Avaliar as alterações em 3D do espaço aéreo faríngeo frente aos procedimentos de cirurgia ortognática de avanço bimaxilar em pacientes Classe I e II esqueléticos. Material e Métodos: A análise da área axial mínima e do volume da aérea superior foi realizada em pré-operatório (T0) e pós-operatório (T1) de 56 pacientes, sendo 21 do sexo masculino e 35 do sexo feminino, com média de idade de 35,8 (±10,7) anos, submetidos ao avanço bimaxilar pela técnica da osteotomia sagital de mandíbula bilateral associada ao avanço de maxila por meio de osteotomia Le Fort I. As avaliações foram feitas através de tomografia computadorizada Cone-beam, utilizando-se o Programa Dolphin Imaging 11.7. Foi utilizado o teste t pareado para comparar os dados pré e pós-operatórios. Todos os testes foram realizados com o programa Statistica, adotando-se um nível de significância de 5%. Resultados: No estudo do erro do método, não houve erro casual nem sistemático entre a primeira e a segunda medição das variáveis (p >0,05 em todas as medidas). A cirurgia de avanço bimaxilar apresentou uma média de 73,6% (± 74,75%) de aumento volumétrico e 113,5% (±123,87%) de aumento na área axial mínima. Conclusões: Podemos concluir que a cirurgia de avanço bimaxilar proporciona um aumento volumétrico significativo no espaço aéreo superior, bem como na área axial mínima, no entanto, esse ganho nem sempre ocorre na mesma magnitude para todos os pacientes.(AU)


Introduction: Facial aesthetics dissatisfaction is considered the most common motivating factor in the search for orthognathic surgery. This procedure may be used in cases of severe dental and skeletal discrepancies in adult patients. The restricted space anatomy of the upper airway space (UAS) allows features such as obesity, muscular hypotonia and mandibular deficiency favor clogging, which may lead to obstructive sleep apnea (OSA), characterized by recurrent episodes of partial or complete obstruction of the UAS during sleep. Surgeries of bimaxillary advancement are associated with increased UAS, however, the morphological and volumetric changes are not well known. Objectives: to evaluate changes in 3D pharyngeal airway in front of orthognathic surgery procedures of skeletal Class I and II subjects. Material and Methods: 3D pharyngeal airway was evaluated preoperative (T0) and postoperative (T1), with the aid of the analysis of the minimum axial area and airway volume. Fifty-six patients 21 male and 35 female, with a mean age of 35.8 (± 10.7) years undergo bimaxillary advancement by the technique of bilateral sagittal split osteotomy of the mandible associated with maxillary advancement through Le Fort I osteotomy. Measurements were made using Cone-beam Computed Tomography, using the Dolphin Imaging program 11.7. Paired t test was used to compare to the data between T0 and T1. All tests were performed with the Statistica Program, adopting a 5% significance level. Results: In the method error of the study, there was no casual or systematic error between the first and second measurement variables (p > 0.05 for all measures). The bimaxillary advancement surgery showed an average of 73.6% (± 74.75%) of increase in volume and 113.5% (±123.87%) increase in the minimum axial area. Conclusions: We concluded that the maxillomandibular advancement surgery provides a significant increase in volume in the UAS as well as the minimum axial area; however, this gain is not always in the same magnitude for all patients.(AU)

Introdução: Dependendo da magnitude da má oclusão de Classe III, esta é uma alteração difícil de ser tratada apenas com a correção ortodôntica. Tanto as cirurgias de recuo mandibular quanto as bimaxilares promovem uma melhora na oclusão, na função mastigatória e na estética facial, ao corrigirem as posições da mandíbula e/ou maxila, mas um importante aspecto da cirurgia ortognática, que não pode ser negligenciado, são os efeitos que os movimentos esqueléticos das bases ósseas podem provocar na região das vias aéreas, ao alterar a posição do osso hióide e da língua. O estreitamento das vias aéreas superiores (VAS) pode comprometer o sono dos pacientes submetidos à correção cirúrgica e predispor ao desenvolvimento da apneia/hipopneia obstrutiva do sono (AOS). Objetivos: O presente trabalho tem como objetivo avaliar as alterações de volume e área axial mínima do espaço aéreo faringeo em pacientes com má oclusão de Classe III esquelética, submetidos à cirurgia ortognática bimaxilar, pela técnica de osteotomia Le Fort I da maxila e osteotomia sagital bilateral da mandíbula. Material e Métodos: As avaliações foram feitas em tomografias computadorizadas Cone-beam, utilizando-se o Programa Dolphin Imaging 11.7. As tomografias de 50 pacientes, de ambos os sexos, com média de idade de 33,40 (± 9,38) anos, foram analisadas nos períodos pré e pósoperatório e as medidas de volume e área axial mínima foram mensuradas. Foi utilizado o teste t pareado e os testes foram realizados utilizando-se o programa Statistica 7.0, adotando-se um nível de significância de 5%. Resultados: ao calcular o erro do método, não foram encontrados erros casuais e nem sistemáticos (p> 0,05 em todas as medidas). As cirurgias bimaxilares para correção da Classe III esquelética promoveram um aumento de 16,68% (±22,61) no volume e 23,58% (± 31,46) na área axial mínima. Conclusões: Mesmo que os efeitos da cirurgia de avanço maxilar e recuo mandibular sobre as vias aéreas não sejam completamente previsíveis, podemos observar que a maioria dos pacientes não apresentaram prejuízos na anatomia faringeana que resulte em diminuição do volume aéreo e área axial mínima, predispondo-o ao desenvolvimento da AOS.(AU)


Introduction: Depending on the extend of Class III malocclusion, it becomes difficult to be treated only with orthodontic correction. Both mandibular setback surgery as bimaxillary surgery, promote an improvement in occlusion, masticatory function and facial aesthetics, correcting the position of the mandible and/or maxilla. But an important aspect of orthognathic surgery that cant be overlooked, are the effects that the skeletal movements of the bone bases causes in the airway space, since they change the position of the hyoid bone and tongue. The narrowing of the pharingeal airway space (PAS) may impair the patient's sleep and predispose to the development of obstructive sleep apnea (OSA). Purpose: This study aims to evaluate surgical changes in the airway volume and minimal cross-sectional area in the pharyngeal airway space (PAS) in patients with skeletal Class III malocclusion, submitted to bimaxillary surgery, using a Le Fort I maxillary osteotomy and bilateral sagittal split ramus osteotomy technique. Material and Methods: The evaluations were made through Cone-beam computed tomography (CBCT), using Dolphin Imaging program version 11.7. The CT scans of 50 patients of both genders, with a mean age of 33.40 (± 9.38), were analyzed pre and postoperatively and volume and minimum axial area were measured. Paired t test was used and tests were performed using Statistica 7.0 software, adopting a 5% significance level. Results: Method error were done and no random or systematic errors were found (p> 0.05 for all measures). Bimaxillary surgery for skeletal Class III correction promoted an increase of 16.68% (± 22.61) in volume and 23.58% (± 31.46) at the minimum axial area. Conclusion: Even if the effects of the maxillary advancement and mandibular setback surgery on the airway are not completely predictable, we observed that most patients didnt have pharyngeal airway anatomy damage, that could result decreased on airway volume and minimum axial area predisposing to OSA development.(AU)

ABSTRACT

Among various surgical methods introduced to optimize esthetic results, robotic surgery has gradually expanded in scope. As incision, approach, and operation view in robotic surgery differ from existing surgical methods, we should consider reconstruction from a different perspective. We recently experienced two mandibular reconstruction cases after tumor ablative surgery with robotic neck dissection using the conventional reconstruction method and virtual surgical planning (VSP), respectively. We found that the conventional reconstruction method is inappropriate in modified facelift incision in robotic neck dissection because it provides limited surgical scope, restricts access to the defect area, and therefore, consumes considerable time before anastomosis. For these reasons, the authors consider VSP far more viable in the era of robotic surgery.

ABSTRACT

Aneurysmal bone cysts are rare benign lesions of bone tissue, infrequent in the craniofacial skeleton about other structures like long bones or the spine. The rare jaw lesions encountered in the mandible and maxilla. On the other side, it is at the same time very exciting in terms of its differential diagnosis with other types of mandibular or maxillary bone lesions. We present the case of a 45-year-old female patient with an aneurysmatic cyst located at the left angle of the mandible describing the treatment for the same i.e. surgical excision and hemi mandibulectomy with reconstruction. We have focused on the differential diagnosis, mainly with the malignancies, which can be found at this location, along with therapeutic options classically described for these kinds of pathologies. In our patient, the surgical excision and hemi mandibulectomy allowed a complete removal of the lesion and recon plate was placed for rehabilitation.

ABSTRACT

La enfermedad de Albers-Schõmberg u osteopetrosis es un raro padecimiento óseo. La osteomielitis mandibular es una infrecuente complicación de esta enfermedad. Por ello y por lo interesante de la presentación clínica de ambas entidades, nos propusimos como objetivo presentar este caso. Este paciente masculino de 32 años con antecedentes de enfermedad de Albers- Schõmberg de tipo adulta benigna que dos años atrás acudió a consulta y refirió haberse realizado una exodoncia once meses antes, a partir de lo cual presentó aumento de volumen facial y dolor de intensidad variable con períodos de agudización. Luego del estudio clínico e imagenológico se llegó al diagnóstico de una osteomielitis mandibular como complicación de su enfermedad de base. Se aplicó terapia antimicrobiana, exéresis del hueso necrótico y oxígeno hiperbárico. A los dos años acudió a consulta con una reagudización del proceso, se le produjo la expulsión espontánea de un secuestro óseo y se decidió la realización de una mandibulectomía parcial más reconstrucción con placa mandibular. En estos momentos el paciente no presenta signos de reagudización del proceso y se mantiene bajo vigilancia estricta a los tres meses de realizada la intervención quirúrgica(AU)


Albers-Schomberg disease or osteopetrosis is a rare bone condition. Mandibular osteomyelitis is an infrequent complication of this disease. It was for this reason, alongside the peculiar clinical presentation of both entities, that we decided to present this case. A male 32-year-old patient with a history of benign adult Albers-Schomberg disease attended consultation two years ago and stated to have undergone exodontia eleven months before, after which he had had facial swelling and pain of variable intensity with acute episodes. Clinical and imaging studies led to the diagnosis of mandibular osteomyelitis as a complication of his underlying condition. Treatment consisted of antimicrobial therapy, exeresis of the necrotic bone and hyperbaric oxygen. Two years later the patient came back to consultation with a relapse of the acute pain process. Spontaneous expulsion of an osseous sequestrum was performed, followed by partial mandibulectomy and reconstruction with a mandibular plate. At this moment the patient does not present any signs of relapse of the acute pain process. Three months after surgery, he remains under strict surveillance(AU)

ABSTRACT

El carcinoma odontogénico de células claras es un raro tumor odontogénico que se presenta principalmente en la parte anterior de la mandíbula, mayormente en mujeres entre la quinta y séptima década de vida. Se caracteriza histológicamente por redes de células con citoplasma claro mezcladas con células que contienen citoplasma eosinófilo. Es sumamente agresivo y puede presentar metástasis local o a distancia. Al emplearse el tratamiento por enucleación y curetaje, frecuentemente da lugar a recurrencia, por lo que se aconseja el tratamiento radical de resección mandibular hasta una zona libre de lesión. El carcinoma odontogénico de células claras debe ser considerado en el diagnóstico diferencial de tumores mandibulares que presenten células claras para el establecimiento del plan de tratamiento adecuado. Se debe incluir el seguimiento a largo plazo del paciente. Se presenta el caso de un paciente femenino de 69 años de edad, con una lesión presente en la región presinfisaria derecha, sin afectación de ganglios linfáticos regionales o metástasis aparente. Se efectuó osteotomía segmentaria mandibular y reconstrucción con placa de titanio(AU)


Clear cell odontogenic carcinoma is a rare malignant odontogenic neoplasm, with a predilection for the anterior segment of the jaws, and presenting basically in women between the fifth and seventh decades of life. CCOC is composed histopathologycally by strands or groups of cells showing clear cytoplasm associated with cells showing eosinophilic cytoplasm and some times with tall columnar cells that resambled ameloblasts. CCOC is considered a low-grade malignancy but in some cases its behavior is aggressive giving local and distant metastasis, so the recommended treatment is a radical resection and follows the patient very close. CCOC should be diferenciated microscopically from other local and metastatic lesions that are composed with clear cells also, in order to render a proper diagnosis and treatment. We are presenting a clinical case corresponding to a 69 years old female patient, with a 10 year history of a slow growing anterior mandibular lesion without lymph node or distant metastasis, with a biopsy diagnosis of clear cell odontogenic carcinoma, the patient was treated by partial resection of the anterior mandible and reconstructed with a titanium plate(AU)
